Welcome! Please fill out our intake form and upload a photo copy of your pet’s rabies paperwork. If under 4 months, a minimum of two rounds of puppy shots will be accepted.

Send current rabies paperwork to 707-740-2044 or TransfurmationsLLC@gmail.com . If everything on file is up to date, feel free to submit your scheduling request here. Please note this is tentative availability. Depending on the size of your pet, desired service, and our current time spots available, your preferred date may not be available. In the notes section, please provide your availability along with desired service.

Online requests are inquiries. The requests are not set in stone until an appointment is confirmed by us. Be sure to send the rabies paperwork (or if under 4 mos, the proof of distemper, parvo, and bordetella vaccines). Failure to do so will result in declining your request.
