The Liability Waiver must be signed prior to the appointment. Please let us know if there has been any changes to your pets health since the last visit.

Any cancelations 7 days prior to the appointment will have a full deposit refund or you can have a 1 time transfer for an alternative clinic, otherwise the deposit is non-refundable.
